Please bring your child with you to registration for a vision, weight, and height screening.
Please bring the following documents:
- Long Form Birth Certificate
- Social Security Card
- Proof of Residency in Pageland (utility bill, mortgage, or lease agreement)
- Proof of Income (W-2, tax return, or paystubs)
